W-2C YES Top part of form has been reformatted, requiring a new envelope for 2009. Boxes 1 through 17 have been moved vertically due to addition of new headings and change in size for boxes 14 and 12d.
W-3C YES Top line of form has been reformatted: Box a has been moved.
1042S MAYBE Size of Box 5 changed: Boxes 6, 7, 8, and 9 moved a small amount.
1096 MAYBE Check boxes for 5498, 5498-ESA, and 5498-SA moved. Check boxes for 3921 and 3922 added.  Other check boxes are in the same position as on the 2008 form 1096

Added boxes 12a, 12b, 13a, 13b, 13c, 14a, a4b, 15a, and 15b.  Height of Boxes 5 and 6 increased.  Height of Box 7 decreased and check box locations are moved horizontally. Layout of entire left size of form changed to accommodate new 1/2 page format.

 The "non-citizen national" check box has been removed. All entries below name and address boxes have been moved vertically. U.S. Citizenship and Immigration Services web site (www.uscis.gov) declares that the correct I-9  form Edition Date is "Rev. 02/02/09. The revision date can be found on the lower right hand corner of the form. No previous editions accepted.".
